We are excited to see you for your appointment! If you are new to Pure Spa, please fill out the appropriate form specific to your treatment. All information provided is confidential and will not be shared with any third parties. Please note that guests receiving facials are required to fill out both the aesthetics form and the sauna/steam room waiver.
